Entry Procedure for Academic Collaborators

A. Notice of Attendance
1. The invited academic collaborators are requested to submit the notice of attendance by Friday, 15th October, 2004.

2. Documents required
a. Notice of Attendance (Form C)
b. Photograph One sheet of attendeefs photograph should be attached to the Form C.
The photograph should be square, preferably trimmed to 4cm~3cm, full face, frontal view, and head-shot.
c. Photocopies of all pages of nomineefs passport that describe the attendeefs status. One copy

3.  If you need the permission of your supervisor to attend the workshop, please let us know as soon as possible. AUICK will send a letter to your supervisor requesting permission for your attendance.
 
B. Procedures for Entry to Thailand

1. Within two weeks of AUICKfs receipt of the notice of attendance, the necessary documents for visa application will be sent directly to the attendees.

2. The attendees should immediately apply for an entry visa at a Thai consular office in their country.

Note: Expenses for obtaining passport and visa will not be reimbursed.
